Repetitive Transcranial Magnetic Stimulation (rTMS) is a form of neuromodulation that uses magnetic pulses, delivered through the scalp, to stimulate activity in specific, targeted brain circuits.
How Magnetic Stimulation Works
An electromagnetic coil placed against the scalp generates rapidly alternating magnetic fields. These fields pass painlessly through the skull — magnetic fields, unlike electrical current, are not significantly impeded by bone or tissue — and induce small electrical currents in the underlying brain tissue, causing neurons in that region to activate.
The Dorsolateral Prefrontal Cortex (DLPFC)
The DLPFC is a brain region strongly implicated in mood regulation, and is found to be relatively underactive in many people with depression. It is the most common target for rTMS in depression treatment, and is also connected to deeper limbic brain structures involved in emotional processing, meaning stimulation of the DLPFC can influence activity throughout this broader mood-regulating circuit.
Neuroplasticity
Neuroplasticity refers to the brain's capacity to reorganise and strengthen connections between neurons in response to activity and experience. Repeated stimulation sessions over a structured course of rTMS treatment are thought to promote lasting neuroplastic changes in the targeted circuit, which is why treatment is delivered as a structured course over several weeks, rather than as a single session.
